
Croydon has nearly 5000 patients with a diagnosed mental health condition.
GPs and Healthcare workers recognise that some patients might feel anxious coming into their registered GP practice or might not be physically able to come to a practice for various reasons. This anxiety was heightened with the pandemic and GPs in Croydon saw a decrease in patients attending for check-ups.
As a result, we have designed a flexible service which means we can come to you!
A Healthcare assistant can visit you at home and carry out all your annual checks which you would normally have done at your GP practice. You have the comfort of your own home and a longer appointment to help with any additional requirements that might need further input based on your need, such as support with finances from a social prescriber or being signposted to social events within the area for anyone who may feel isolated or just wants to make new friends.
We appreciate that not everyone is comfortable being seen at home and if this is the case then just let us know when we get in touch to book your appointment and we can arrange a time for you to be seen at your GP Practice instead.
